2-Part Universal Saliva & Urine Drug Test Result Forms, 25/Package

Note: Abbott Toxicology has discontinued Forms 2430P and 2440P and replaced them with the universal 2450P form.

The numbered, paper pack is a 2-Part carbon-less form with which provides a carbon-less copy for the specimen donor. Twenty-five forms per pack.

How to Complete a 2-Part Result Form or Photocopy Template

1. Write in company and donor information.
2. After the urine specimen is collected and the drug test device is sealed with a security seal, the donor signs the form and provides their contact information. At this point, a copy of the form is to be given to the donor, who may now be dismissed.
3. When the drug test results are available, the collector fills in test details such as Lot # and Expiration Date and records the test results.
4. The collector signs the form and records the time and date of collection.
